One connected pathway. Clear roles. Human support.
Step 1
Identify
Included Health recognizes that a member's healthcare issue may be affecting work, income, insurance coverage, or government-benefit eligibility.
- Member directly asks for help
- Care Advocate identifies a potential need
- Member selects a relevant digital-support topic
- Employer or health plan sends approved educational outreach
This does not involve automated medical surveillance.
Step 2
Explain
The member receives a plain-language explanation of available support.
“Based on what you shared, you may benefit from speaking with a specialist who understands disability benefits, Medicare, and work-transition programs. Would you like us to connect you?”
Step 3
Consent
The member chooses whether to proceed. Consent is explicit, and the member sees exactly what limited information will be shared.
- Name
- Contact information
- State
- Employer or plan affiliation
- General support category
- Preferred contact method
Clinical information is never shared automatically.
Step 4
Connect
Three potential referral models could apply, depending on the member's need and the pilot's design.
- Model A: Digital referral
- Model B: Warm transfer
- Model C: Scheduled specialist appointment
Step 5
Support
Allsup provides the appropriate specialty service — disability guidance, SSDI support, Medicare transition help, or veterans appeal assistance.
Step 6
Close the loop
With member permission and appropriate agreements, a limited status may return to Included Health.
- Referral received
- Member contacted
- Consultation scheduled
- Member declined further support
- Service engagement started
No detailed claim or medical information is displayed.
Clear data boundaries, by design.
Included Health data
Clinical navigation records stay within Included Health's systems.
Member-consented referral information
Only what the member explicitly agrees to share moves across the boundary.
Allsup service data
Case and service data related to Allsup's specialty support stays within Allsup's systems.
Shared limited status information
A small set of high-level statuses — never detailed claim or medical data — may flow back.
Core principles
Business-associate and data-sharing obligations are to be determined. A production partnership would require legal, privacy, security, regulatory, and contractual review.
